Welcome to Homebound Health and Wellness. We provide concierge primary care for those with limited mobility residing in Assisted Living, Personal Care or Group Homes. We believe that access to primary care should not cause undue stress. We limit your travel to office visits by bringing primary care to you.
Keeping Your Health in Mind
Initial visits are conducted in the comfort of your home. Telemedicine visits are only offered to established patients at this time. Please call us to schedule an appointment, or schedule it online using our scheduling tool. If you are having an emergency, please call 911.
